Reverse Mapping PDF Forms (Using Forms as Intake to Populate AMS)
  Reverse Mapping lets you use a fillable PDF (ACORD or custom) as an intake: when the client/staff fills it, Momentum AMS reads the form fields and creates/updates records (Contacts, Insureds, Policies, etc.).

 Important: Reverse mapping also requires a fillable PDF. If the form fields don’t exist, AMS can’t “read” the values. Convert your PDF to fillable before mapping (see below).

 Use this when you want returned forms to feed data back into AMS. If you only need to send a pre-populated document, use Mapping Data to PDF Forms (linked below). 


1️⃣ Open the Reverse Mapping tool

  • Go to Agency Profile → Other → Reverse Mapping PDF Forms.

  • Click Add New.

2️⃣ Select or upload the intake form

  • Choose Upload Your Own Form for your custom fillable PDF or Select from Existing Form.

  • If not fillable yet, make it fillable first (Acrobat → Prepare Form, add fields, save, then upload).

3️⃣ Mapping Fields 

  1. When you are mapping a PDF form in Momentum AMS:

    • Click directly on the fillable field in the PDF preview — for example, the “Name of Insured” field.

    • In the mapping panel, navigate through the AMS data categories (e.g., Insured, Policy, Vehicle) to find the field you want to connect to.

    • For Name of Insured, you would select:
      Insured → Name

    • For Address Line 1, you would select:
      Insured → Address 1

    • Repeat for each field until all necessary form fields are linked to their AMS data counterparts.

  2. Once saved, any time you use this form, the mapped AMS data will automatically populate those fields (for PDF Form Mapping) or the entered form data will create/update the correct fields in AMS (for Reverse Mapping).

4️⃣ Using a Reverse Mapped Form 

  • From the form list, use Actions to:

    • Edit – Adjust mapping or upload a new version of the form.

    • Fill the Form – Enter data into the form to create a new insured/prospect in the AMS and prefill related data like vehicles and drivers.

    • Integrate – Generate iframe code to embed the form on your agency’s website. Any submitted data will go directly into the AMS and appear in PDF Forms Reverse Mapping – Filled (accessible from your AMS dashboard or Agency Profile).

5️⃣ Merging Data from a Reverse Mapped Form 

Once your Reverse Mapped form is set up and you’ve received a completed version, you can merge its data into Momentum AMS.

  • Access the Completed Form
    Go to PDF Forms Reverse Mapping – Filled from your AMS Dashboard or from the Admin → Agency Profile menu.
    Locate the completed form you want to process.

  • Open and Edit
    From the Actions menu to the left of the form, click Edit.Scroll to the bottom of the form.

  • Merge from Form
    Click Merge from Form.

    • If the email address in the form matches an existing record, the system will attempt to merge into that record.

    • Warning: This can overwrite existing data in the AMS. 


Best practices

  • Keep required set minimal. Too many required fields cause intake failures.

  • Standardize formats. Use consistent date/phone formats and tell clients not to alter field labels.

  • Pilot first. Test with an internal sample before sending to clients.

  • Version + owner. Add Owner and Next Review date in the mapping notes.
